Information for patients regarding NHS Prescription Charges / Exemptions

Excluding patients under 16 or over 60

As we provide pharmacy services remotely we will complete the declaration on the reverse of your prescriptions on your behalf. Please let us know your current exemption status or if you pay for prescription charges. We will add notes to your file.

We ask you to provide evidence of your exemption by sending a photo of your evidence and emailing it to pharmacy@nowhealthcaregroup.com. We will add this to your file.
